ABOUT THE CWC: CCAD's Counseling & Wellness Center (CWC) is seeking an energetic and creative mental health professional to fill the role of Care Coordinator. The CWC staff is a highly collaborative and cohesive team. We provide high quality, compassionate mental health services for students, and collaborate with community partners to implement relevant and engaging outreach and training programs. We believe in students' resilience and strength, and we honor diversity, equity, and inclusion in our work. Our goal is to support students in their natural ability to thrive at CCAD and beyond, and to inspire a creative culture that prioritizes and fosters whole-human health, education, and growth.


SUMMARY

The Care Coordinator's direct service responsibilities primarily include responding to student referrals from various campus partners to establish mental health care needs, linkage, and support to students that are experiencing difficulties within the campus setting. This may include holding office hours for students to seek services, assisting Safety & Security with mental health triage, finding appropriate care within the college or in the community for the student, and campus program development. Percentage of time spent in job duty responsibility categories may shift based on current needs of the CWC. Typical hours of operation are between 8:30am and 5:00pm, Monday-Friday, with potential moderate flexibility.

This is a temporary, grant-funded full-time position ending June 2025 with opportunity to be renewed based on professional and organization fit and needs of the organization.

ESSENTIAL DUTIES AND RESPONSIBILITIES include but are not limited to the following:

Direct Service

  • Facilitate student linkage to community resources such as medical appointment scheduling/paperwork, contacting outside providers for refills/appointments, reducing food/housing insecurity through community resources, locating gender-affirming care, insurance navigation/application, utilizing CCAD resources, vocational opportunities, and recreational activities.
  • Follow up on campus referrals for case management services.
  • Assist students in non-clinical skill development including but not limited to interpersonal skills, time-management, life skills, ADLs, etc.
  • Meet with students in distress to link to appropriate resources such as Counseling & Wellness, Dean of Students, Safety & Security, hospitalization.
  • Partner with campus resources to help students better understand opportunities for assistance and ensure academic success.
  • Build connections with community resources such as medical, dental, gender-affirming care, psychiatric care, housing and food resources.
  • Serve as a member of the CCAD Care and Response Team and provide outreach for mental health related Care Reports.
  • Manage the Community Cupboard (small, on-campus food pantry).
  • Provide support and outreach after students return from hospitalization.

Administration

  • Track and respond to incoming CARE reports for students.
  • Participate in regularly scheduled CWC team meetings
  • Participate in weekly Care and Response Team meetings.
  • Participate in monthly Student Development Staff meetings.
  • Maintain professional development to stay knowledgeable regarding college mental health, changing trends, best practices, legal and ethical issues, or specific populations/clinical considerations.
  • Assist in other administrative functions or tasks, as requested.

Outreach Programming

  • Assist in development, implementation, and coordination of outreach events including "Adulting" classes and other position specific events.
  • Under supervision of the Director, provide support groups for identified students (clinically appropriate groups such as ADLs, social skills, meditation, etc.)
  • Develop and distribute appropriate marketing materials.
  • Attend orientation events as requested to provide linkage to the Counseling & Wellness Center.

QUALIFICATIONS:

  • Evidence of strong organizational skills
  • Are able to deliver engaging and innovative outreach, wellness, or training programming.
  • Demonstrate a record of teamwork and creativity.
  • Are able to operate within an electronic record management system (Advocate/Titanium), and demonstrate technology literacy and competence.
  • Are self-motivated and able to work within flexible and fast-paced environments.
  • Are down-to-earth and highly relatable with college students in therapeutic and programmatic contexts.

PREFERRED QUALIFICATIONS:

  • Have experience working in a collegiate setting.
  • Demonstrate appreciation and knowledge of the process of intersectional identity development.
  • Have experience and competence in the treatment of trauma-related, comorbid, or severe mental health concerns.
  • Experience and competence in working with people of color.
  • Experience and competence in working with LGBTQIA+ identifying individuals, especially clients identifying as transgender, gender-queer/fluid, non-binary, or other gender identities.

EDUCATION and/or EXPERIENCE

  • Bachelor's degree in Social Work, Psychology, Case Management, Human Development, or similar degree is required.
  • Experience with case management.

CONTRACT and COMPENSATION

This is a temporary, grant funded full-time employee position, with a contract period end date.

  • The employee must occasionally lift and/or move up to 25 pounds.
  • Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to focus.

WORK ENVIRONMENT

  • The noise level in the work environment is moderate.
  • The pace of the work environment is moderate to fast.
  • The employee is occasionally exposed to outside weather conditions.
